The following questionnaire was finalised following consideration by the experts at the ePSIplus Financial Impact (PSI Pricing) Thematic Meeting 3 meeting held in Athens on the 20 - 21 November 2008. It was used to conduct the ePSIplus PSI Pricing Survey.

The purpose of the questionnaire was to support an ePSIplus Thematic Network survey that opened on 1 December 2008 and closed on 26 January 2009.
